There’s no doubt you’ve had patients in your office who you worry about the second they leave. Thoughts might be “Is their home environment safe?”, “Are the caregivers really involved enough?”, or even “Can they handle this diagnosis on their own?”

This is where Monarch comes into play. Trained, compassionate nurses, physical therapists, occupational therapists, speech therapists, medical social workers, and certified home health aides take off where you’ve left off.

Comprehensive Assessment
When you refer a patient to Monarch, within 48 hours a comprehensive home health assessment will occur. At this assessment, a registered nurse or physical therapist will assess the safety of a patient in their home environment, the management of their medical condition, and the presence of trainable caregivers. Any disease management teaching, additional discipline assessments, or even outside referrals such as county social work and caregiving (IHSS) or congregate living placement will be fully addressed.

Communication
These are your patients, with your supervision ultimately following the patients throughout the care continuum. So what Monarch will do is represent you in the manner expected of you and your office, while also communicating vital updates back. If you prefer Emails, texts, or calls –
we’ll make it happen.

Common Referrals
Diabetes management – You do a great job of telling your patients what matters when it comes to diabetes management. But are regular foot checks really happening? Are they monitoring their blood sugar levels and/or taking insulin at the appropriate times? How
about following their diet? And when it comes to activity, are they truly living their best life, or are they remaining sedentary? Monarch’s team can answer these questions.

Post-CVA – When your patients experience the trauma of a CVA, we hope you will refer them to Monarch to provide nursing, speech therapy, physical therapy, and occupational therapy. For these patients we’ll pull out all of the stops, and while we’re not miracle workers, the results can be truly amazing if implemented early on.

Wound management – Wounds are easy when addressed properly. That’s why Monarch’s nursing team can be ordered to teach patients and caregivers how and when to manage surgical, pressure, stasis, or other wounds. Taking it a step further, the
rehabilitative team can ensure the patient is able to safely and fully complete tasks despite their wounds.

Respiratory care – If there’s one thing we learned from covid, besides the importance of hand washing, it should be an appreciation for what it means to breathe fully and freely. Unfortunately, patients with PEs, COPD, and other respiratory conditions may struggle to perform their daily activities without becoming winded. Monarch’s care team can provide the teaching and training necessary for patients to regain independence with
decreased dyspnea.

Rehabilitation – As we age it’s easy for various diagnoses, perhaps diabetes, high blood pressure, or other disease to result in inactivity. Over time this can build up and be difficult to get out of alone. Surgical procedures such as a recent knee or hip fracture can also cause painful contractures which limit movement and increase the chances of ffalling. Whatever your patient’s rehabilitative needs are, Monarch’s trained physical and
occupational therapists can get them out of their rut.

Home Safety – Maybe your patient is telling you about all of their dogs. Or briefly mentions how they’ve been struggling to take a shower. Their frail condition might even be a factor. Whatever it is, Monarch is happy to send a qualified clinician out to perform a home safety assessment. And if everything is good, then at least you have peace of mind knowing that your patient is going to live more years with the independence they deserve.
